If your ballot was not counted because of a problem with the signature on the ballot return envelope, you can fill out a simple form and send it to the county to "cure" the problem. You can even return the form by email.

  • California driver license or ID card number

  • Last four digits of social security number

  • Date of birth

Your information will be provided to the California Department of Motor Vehicles (DMV) who will retrieve a copy of your DMV signature electronically for the purpose of registering to vote.
